The conditions listed below need regular follow up and treatment to ensure health, well-being, and long life!

Our nurses run clinics for all these and we recommend that patients attend regularly.

Influenza immunization

We recommend a yearly 'flu jab' to the elderly and patients with chronic heart, lung or kidney disease and those suffering from diabetes. This includes diabetic and asthmatic children and young adults. Also, residents of nursing and rest homes are advised to have this injection. The flu vaccine is usually available at the beginning of October but this year it has been delayed and therefore our Flu Saturday will be held on 12th November 2005.  Our three surgeries will be open on that day from 09:00 to 12:00 and no appointment is necessary - just turn up!

Diabetes

Our nurses run these clinics to a protocol.

All known diabetics should attend for the following follow up. This is important as it has been proved that rigorous surveillance of diabetics minimises diabetic complications. 

Most diabetic patients only attend the surgery for check ups. Some patients attend the hospital clinic but we suggest these patients also attend our clinic.

  • 4-monthly check. Book with phlebotomist for blood tests. See the nurse a week later for results and her examination and assessment. Please bring any blood or urine recording books with you.
  • Annual check. See the nurse for blood tests and a thorough assessment. Please bring any blood or urine recording books with you. See the doctor a week later for interpretation of all the data and consideration of an annual plan.

Every year diabetics will receive an invitation to have retinal photography.

Please attend an optician annually.

All diabetics should be under the care of an NHS chiropodist.

Patients are referred to the dietician at the discretion of the doctor.

Asthma

Our nurses run these clinics to a  protocol.

All asthmatics should attend nurse asthma clinics annually at least. If you are recording your own peak flows, please bring your recording book. Please bring any inhalers you use.

The practice has access to spirometers. These machines give a very accurate assessment of lung damage. If you are a smoker, why not have spirometry? You may be convinced to stop!

COPD - Chest Disease

Our nurses run these clinics to a protocol.

All patients with chronic lung disease (COPD - chronic bronchitis, emphysema) should attend nurse asthma clinics annually at least. If you are recording your own peak flows, please bring your recording book. Please bring any inhalers you use.

The practice has access to spirometers. These machines give a very accurate assessment of lung damage. If you are a smoker, why not have spirometry? You may be convinced to stop!

Hypertension

Patients with high blood pressure are invited to attend the nurse for regular blood pressure checks. Nurse works to a protocol and the interval between visits depends on the level of control and the protocol.

Heart Disease

We are active in following up our heart patients. We try to stick to local protocols. We are keen to prevent at risk patients from developing a cardiac problem in the first place. Those who suffer cardiac problems are monitored for:

Diet, Exercise, Stress, Alcohol, Smoking, Cholesterol, Blood Pressure, Diabetes as well as the condition itself and its medication.
