The Rowcroft Specialist Palliative Care Team work closely with Rowcroft Hospice and the Palliative Care Team based at Torbay Hospital.

OUR AIMS

The Community Specialist Palliative Care Team offers specialist advice and support for people with cancer and other illnesses.

OUR PHILOSOPHY

We are a community based team working with patients and their carers in their own homes. 

The team comprises:

Clinical Nurse Specialists
Occupational Therapists
Consultants in Palliative Care
Social Workers
Secretary

We offer specialist symptom control advice and support.  We aim to work in partnership with patients, carers the Primary Health Care Team and other agencies involved.

WHAT YOU CAN EXPECT FROM US
  • Support for you, your partner, family and friends by telephone or visits to your home
     
  • Specialist advice about any symptoms you may experience
     
  • Access to other parts of the service, eg:

    Specialist medical support
    In-patient unit
    Day Hospice
    Lymphoedema management
     
  • Specialist advice for anyone involved in your care
     
  • Help and guidance with financial or social problems
     
  • Liaison with other agencies involved in your care and other services that may be able to help
     
  • We work closely with your GP and Community Nurse to help provide the best care possible

The Team is based in at Rowcroft Hospice.

Regular hours of work are from 9.00am to 5.00pm Monday to Friday.  If we are not available please leave a message or we can be contacted through our secretaries as follows:

At weekends and Bank Holidays between 9.00am to 5.00pm a Macmillan Nurse is available fore telephone advice on

OUT OF HOURS ADVICE

After 5.00pm you can ring the nurse in charge at Rowcroft Hospice for advice on 01803 210800.  They will be able to talk a problem through or give advice regarding other services that may be able to help.
